Medical Records Department

The Medical Records department is located on the ground floor of the Geoffrey Hodges building at the Outwoods site. We supply all case notes for the Trust, both in and out patients as well as notes for all other departments and secretaries. We also log referrals, merge double registrations and handle all requests for the Trust regarding access to health records under the Data Protection Act 1998.

The department is open from 8.30am to 10pm every week day. We operate a closed library, to ensure security and confidentiality of our notes whilst held in the department.

The Medical Records Department currently functions across 3 sites, Burton, Sir Robert Peel and Samuel Johnson Community Hospital

Provision of health records for all out patient clinics and admissions

Provision of health records for the purpose of audit

Handling of access to health records requests in line with the Data Protection Act 1998

Logging of referral letters from GP practices

Merging duplicate registrations

Deceased notifications

Archiving and destruction of records in accordance with DOH guidance

How to obtain an appointment 

Patients wishing to view their health records can arrange an appointment by contacting the Subject Access Team on 01283 566333 ext 5208/5209 

For the purposes of audit a room is available and can be booked by contacting the Medical Records Supervisor on ext 5235
